Massachusetts Department of Public Health– Dedicated to providing individuals with serious advancing illness the information about the range of choices for their care so that they and their advocates can make educated healthcare choices. Massachusetts Department of Public Health created a brochure called “Know Your Choices: A Guide for Patients with Advancing Serious Illness” which is translated into Arabic, Cape Verdean, Chinese, Haitian Creole, Khmer, Portuguese, Russian, Spanish, and Vietnamese.
